Mastopexy – Purpose and Degrees

Mastopexy (breast lift) is a surgical procedure aimed at restoring the normal volume of the breasts. This operation alters the size and improves the contours of the mammary glands. The shape and size of the areolas can also be adjusted for better aesthetics. According to plastic surgery statistics, approximately 450,000 mastopexies are performed annually. The main causes of breast sagging are hormonal effects on glandular tissue. The lowering of the mammary glands is called “ptosis.” Normally, the nipple should be above the inframammary fold. Breast ptosis is classified based on the position of the nipple relative to the inframammary fold. There are three degrees of ptosis:

1. First-degree ptosis: The nipple is at or above the inframammary fold.
2. Second-degree ptosis: The nipple is below the inframammary fold (but above the lower contour of the breast).
3. Third-degree ptosis: The nipple is positioned at the lower contour of the breast and points downward.

Plastic surgery also identifies terms such as:

• Pseudoptosis: The nipple is located above the inframammary fold, while the lower part of the breast is sagging due to hypoplasia.
• Glandular ptosis: The nipple is positioned similarly to pseudoptosis, but the breast is full, and its lower portion is significantly drooping.

Types of Mastopexy Procedures

1. Periareolar mastopexy: Used for first-degree ptosis with elongated breasts. This technique involves a circular incision around the areola. Excess skin is removed, and special sutures tighten the breast. Minor skin folds around the incision may appear but usually resolve within two months.
   • Duration: Around 1.5 hours.
   • Anesthesia: Local or general, depending on the diagnosis.
   • Recovery: 1-2 weeks.
2. Vertical mastopexy: A popular method involving incisions around the areola and vertically downward to the inframammary fold. Suitable for first and second-degree ptosis. Excess skin is removed, and the nipple is repositioned to its normal location.
   • Duration: About 3 hours.
3. T-shaped mastopexy: Used for second and third-degree ptosis. Incisions are made in the shape of an anchor, starting around the areola, vertically down, and along the inframammary fold.

Indications and Contraindications
Mastopexy is suitable for patients with sagging breasts. However, women planning to give birth in the near future are advised against this procedure.

  5. Age under 18.
